La partenza è prevista dal villaggio di San Bernardino (parcheggio Moesa o Ostello, con parchimetro) per dirigersi verso il comprensorio sciistico di Pian Cales, presente in centro al paese, dal quale si sale, seguendo la strada che porta all'acquedotto, in zona Pian Lumbrif. Al bivio mantenere il sentiero di sinistra, che prosegue diritto verso il Passo, fino a giungere alla sua sommità. Svoltando poi a sinistra si arriverà ai laghetti (lago Moesola) e all'Ospizio. Durante la stagione estiva l'Ospizio del Passo del San Bernardino è operativo con offerta di ristorazione.
